Patient Portal

Current clients can use our secure patient portal to manage scheduling, billing, messaging, forms, and log in to telehealth sessions. If you don’t have a portal account yet or have forgotten your password, please contact our Care Coordinator at info@azaleainstitute.com or reach out to your therapist directly.

Records Requests

Current clients can request copies of their records through the secure patient portal. Clients without portal access may contact our administrative team to request their records or to obtain portal access.
